The UG Public Health Department will be providing free Childhood Immunizations at their Central Vaccine Site, the former Kmart, located at 7836 State Ave. These free clinics are for families who are uninsured or have Medicaid. Those with private insurance will need to get their childhood immunizations from their doctor.

These Childhood Vaccine Clinics will run from 1:00p - 6:30p - please see dates for specific age ranges below. Families are also welcome to make an appointment for Childhood Vaccines at the Health Department.

Middle School Students: May 19th and 26th
High School Students: June 23rd and 30th
Elementary Students: July 7th, 14th, and 21st

The UGPHD is asking families to bring the HIPAA & Vaccine Consent forms with them prefilled out as to save time on the day of the events (see linked documents below).

It is very important for families to bring either their child's vaccine record or the note from the nurse stating what vaccine(s) they need.
