Staff Transportation Request

This form is to submit a request for transportation. Please note that submitting this form does not guarantee approval. You will be notified within two working days if the request is approved or denied. 
 
Due to route times, Turner Transportation requests that all field trips be scheduled between 8:45a - 1:45p. If a trip has to be scheduled outside of these times, please contact Joe Peterson at petersonj@turnerusd202.org.
 
PLEASE NOTE: You must get your principal/supervisor approval for the field trip prior to submitting this request. If this trip is for students with special needs, this must first be approved by the Office of Special Services. 
 
If you need a THS van, contact Rebecca Gutierrez. If you need a TMS van, contact Jill Kroh. This form is only for school buses and the district suburbans.
