Approved Rider Request Form

This form is for parents/guardians who need their student added back onto a bus route. To be an approved rider, the student must ride the bus at least 50% of the time each school week. 

To have your student returned to their bus route, complete the information below with as much detail as possible. Each student must have their own form submitted (i.e. an individual form for each sibling). The form will be reviewed by a transportation staff member who will contact you within two business days.

We appreciate your patience as we review each request. Please note that submitting an approved rider request does not guarantee your student rider will be placed on the approved rider list.
