Athletic Information

Student Athletes must have the required paperwork turned in before they can participate in practice for all sports. This includes the following:

1. Physical dated after May 1, each school year 

2. Parent Signature for KSHSAA

3. Emergency Medical Authorization/Insurance Information

4. Code of Conduct

These forms are available at the Turner Middle School Office, and the Turner High School office. You Must Use The PHYSICAL FORM Provided By The TURNER ATHLETIC OFFICE For The Physical To Be VALID.

Please remember that athletic physicals for Kansas State High School Activities Association (Turner USD #202 events) sponsored events may not be taken prior to May 1, 2010 to be valid for 2010/2011 school year.
